A Case Manager and Patient Advocate

Every All Progressive hospice nurse serves as both a case manager and patient advocate. They closely monitor vital signs, manage medications, particularly for pain control, and provide ongoing symptom management in collaboration with our physicians and interdisciplinary care team. Through expert nursing, our team ensures that patients experience comfort, dignity, and peace throughout their hospice journey by:
 

• Offering specialized pain management and symptom control

•  Addressing concerns such as physical discomfort, breathing difficulties, and emotional distress. 

• Collaborating with our physician and interdisciplinary care team to ensure that each patient’s care plan is thoughtfully tailored and adjusted to meet changing needs.
